Participant Agreement

Paradise Bound Ministries, Inc., a Michigan nonprofit corporation, and Asociacion Mision Paraiso, a Guatemalan legal entity, are Christian organizations dedicated to ministering around the world (“the Organizations”) and rely on the involvement of volunteer participants to fulfill this mission. In pursuit of the Organizations’ missional work, volunteer participants may travel domestically or internationally, including to and from Guatemala, to provide food, homes, and medical care and engage in activities related thereto (the “Activities”). Before participating in any of these Activities at any location, each volunteer participant must review and sign this Participation Agreement.

In consideration for the opportunity to participate in Activities, the undersigned (“Participant”), hereby agrees to the terms and conditions of this Participation Agreement as follows:

Waiver and Assumption of Risk.

•   By signing below, I acknowledge and agree to the assumption of liability, waiver, release, and indemnification of the Organizations and their directors, officers, employees, contractors, volunteers, agents, and representatives (collectively, the “Released Parties”) as more fully described below.

•   By signing below, I acknowledge and agree that the Activities may be dangerous and participating in the Activities involves risk of various types of injury or illness, including (but not limited to) physical, mental, or emotional injury or illness, property or financial damage, or even death, any of which may arise from the negligence of the Released Parties.

•   By signing below, I acknowledge and agree that I am assuming such risk of injury, illness, damage, or death.

•   By signing below, I acknowledge and agree that I am financial responsibility for any such injury, illness, damage, or death sustained by me or my child, as applicable, during participation in the Activities, including (but not limited to) medical expenses incurred in diagnosing and treating any injury or illness, and I will indemnify, defend, and hold harmless the Released Parties from financial responsibility for any such injury, illness, damage, or death, even if arising from the negligence of the Released Parties.

•   By signing below, I freely and knowingly waive all claims, causes of actions, demands, liabilities, and expenses of every kind and nature (including attorney’s fees and costs) against the Released Parties that I or my heirs, successors, assigns, and next of kin may have now or in the future arising from my participation in the Activities.

  Media Release.

 By signing below, I acknowledge that the Organizations, their directors, officers, employees, contractors, volunteers, agents, and representatives may photograph, audiotape, and/or videotape volunteer participants during their participation in the Activities. I authorize all such photographs, audiotapes, and videotapes and grant the Organizations the royalty-free right to use all photographic images and audio and/or video recordings, and any reproductions thereof, in any of the Organizations communications and/or promotional materials related to activities sponsored by the Organizations.

Medical Authorization

 By signing below, I authorize the Organizations’ personnel accompanying the Participant, whether a director, officer, employee, contractor, volunteer, agent, or representative, to arrange for, and consent to, any medical treatment recommended by the Participant’s attending physician if the Participant is unable to make his or her own decisions regarding medical care at that time.

Terminating Participation.

 By signing below, I acknowledge and agree that either Organization may terminate a Participant’s participation in the Activities at any time, without cost or expense to either Organization, if any of the Participant’s behavior, language, or actions is offensive or detrimental to the Organizations’ ministry or anyone involved in that ministry.
